• You feel that there are significant factual entries that you would like to have changed. Please note that we can only do this for the records you have with your current practice. 

We will address this in the following manner:

• Based on the information you provide, we may be able to amend the record directly. In some cases, we may need to add a supplementary note or comment to your record rather than change the original entry. We may not need to see you in person, but we may contact you to arrange a telephone appointment to discuss your request further.
• We may signpost you to relevant NHS websites or resources that provide further information about the contents of your medical records.
• If your query relates to an appointment, investigation, test result, or treatment that was arranged by a hospital or community service, we may advise you to contact that organisation directly for further information.
• If you believe there is a significant factual error in your medical record that requires correction, please note that we can only amend records held by your current GP practice. Records created by other healthcare organisations can only be amended by the organisation responsible for them.
• There may be instances where we are unable to amend or remove information from your medical record. This may be because the information is considered to be a professional opinion, forms part of the historical record, or does not meet the criteria for amendment. Where this is the case, we will always explain the reason for our decision and discuss any alternative options available to you.
